INTERNAL MEMORANDUM — RE: CLASSIFICATION STATUS ANOMALY

Following a comprehensive post-incident review conducted by the Department of Vital Records Reconciliation, an 18-month-old patient initially categorised as deceased has been reclassified as ‘viable’ pending further administrative assessment.

The child was pronounced dead following a drowning incident and transferred to morgue storage in accordance with standard protocol. During routine inventory procedures on July 8, 2026, the patient was discovered to exhibit metabolic activity inconsistent with the deceased classification. This discrepancy has prompted a full departmental audit.

The patient has since been transferred to a secondary medical facility for continued observation. Hospital administration notes that the initial death determination was made following established diagnostic criteria and was properly documented across all relevant systems. The subsequent discovery of continued life function does not, at this time, constitute grounds for disciplinary action against personnel involved in the original assessment.

We are currently reviewing our intake protocols to determine whether additional verification steps might have prevented this classification error. A task force has been assembled to examine whether our existing documentation procedures adequately account for the possibility of patients being, technically, alive.

The family has been notified of the status change. All relevant paperwork is being updated retroactively. We regret any inconvenience caused by this administrative inconsistency.
