INTERNAL MEMORANDUM — BEACHES AND COASTAL MANAGEMENT DIVISION Date: June 24, 2026 Re: Post-Incident Protocol Review, Coogee Beach Incident (June 16)

Following Ms. Leah Stewart’s emergence from induced coma on June 23, the Beaches and Coastal Management Division has convened an emergency task force to assess current signage adequacy at Coogee Beach. While Ms. Stewart’s condition stabilises following the amputation of her left arm, departmental focus has shifted to whether existing beach safety materials require revision.

A preliminary audit of current safety infrastructure reveals three operational warning signs, two laminated brochures (printed 2019), and one functional loudspeaker system. The Division notes that these materials were compliant with 2019 standards and remain technically functional.

The task force has identified several options for consideration. Option A involves updating the brochure to include updated shark species taxonomy. Option B proposes a new colour scheme for warning signage. Option C recommends a feasibility study into the feasibility of conducting a feasibility study.

Ms. Stewart’s medical team reports she is breathing independently and has begun rehabilitation. The Division anticipates that brochure redesign consultation may commence within eighteen to twenty-four months, pending budget allocation and stakeholder alignment.

All parties acknowledge the incident was unfortunate. It is not yet clear whether the incident was preventable through additional signage, though the Division remains committed to exploring this question through proper channels.
